Referral Procedure
Eligible students (moderate to severe low vision or total blindness) can get a referral to the service through their school.
Schools may request a referral application form from Ebenezer School, and submit the duly completed and signed application form, parent consent form and a copy of the eye report to the Education Bureau (Special Education Support 2 Section) by post or fax.
